Boiler shops, storage tank builders and autoclave manufacturers work with plate and dished heads from the moment they are received and stored. Plate is cut, bevelled, rolled and formed into shells, then shells, heads and nozzles are fitted up, which often means working inside the vessel. Seams are welded with submerged arc, flux-cored and stick processes, followed by weld dressing, heat treatment, inspection and testing. Gases, consumables and chemicals are handled throughout, and the plan has to follow every stage of that build.
The hazard and risk register holds 57 hazards and 453 controls. Plate rolling, pressing and shell forming is rated 5C, since rolls draw material in faster than a person can let go and a shell that springs can crush someone against plant. Welding, grinding and cleaning inside vessels and shells is rated 5B for oxygen displacement by carbon dioxide or argon, fume and ozone at many times open-shop levels, heat stress and a rescue problem that has killed more responders than the original casualties. Cranes, forklifts, fixed plant, toppling assemblies and hot work fires follow.
A generic construction WHS plan does not know what a set of plate rolls is, and it will not treat the inside of a half-built tank as a confined space with its own atmosphere. It has no rescue plan for a welder who collapses behind a manway, and no link between heat treatment, testing and the people standing nearby. Those are the gaps that matter most in a vessel shop.
Clients buying tanks and pressure equipment are usually experienced at reading safety documents, and after the signed policy they turn to how internal work is controlled. The plan's register rates work inside vessels near the top, with entry controls, atmosphere and ventilation arrangements and a written rescue arrangement, and it gives plate rolling its own guarding and procedure.
Fume and ozone inside shells are health hazards, so reviewers look for how exposure is managed and where health monitoring fits. Insurers generally check the emergency plan, including how a person is recovered from inside a vessel, and the hot work controls. The contractor section shows how inspectors, testers and labour hire are prequalified, inducted and supervised, and the tender cover sheet maps each panel question to its answer.
Water authorities, councils and industrial clients commonly let tank and vessel packages through formal tenders, and the WHS section is scored. If your response shows good welding procedures but no management plan, confined work arrangements or rescue plan, the evaluation marks it down and the contract goes to a builder who documented those things.
On your floor, the event people fear most is a welder found unconscious inside a shell after argon or carbon dioxide has pooled, followed by a workmate who climbs in to help and is overcome as well. When you are asked afterwards what your entry procedure and rescue plan were, a habit of keeping someone at the manway is not the same as a written arrangement that was trained and practised.
Plate rolls cause the other serious injuries, typically a hand drawn in while feeding or a shell that springs off the rolls. The regulator will ask for the risk assessment, the operator's training record and when the rolls were last inspected, and without a plant register or incident register you have nothing written to show them.

You must run the system it describes — consult your workers, put the controls in place, keep the registers current and review the plan after any incident or change.
