The documented system, not just the method
A feed mill supplies every condition for a dust explosion as a matter of routine. Every transfer point makes a cloud; every bucket elevator leg, hammer-mill housing, bin and dust collector is a confined volume; bearings, belts, tramp metal, hot work and electrical equipment are ignition sources. Under the WHS Regulation 2025 (NSW) an atmosphere is a hazardous atmosphere where combustible dust is present in a quantity and form that would result in a hazardous area (s 51), the risks from ignition sources in it must be managed (s 52), combustible material must be kept to the lowest practicable quantity (s 53), and an ignition source must not be introduced where it could cause fire or explosion (s 355). A mill that holds a risk assessment and nothing else has documented its hazards and left the zoning, the ignition-source controls, the housekeeping standard and the emergency response undocumented.

A mill cannot leave its elevators and collectors unassessed for that long, and a regulator will not accept a booking confirmation as a control. 10 hazards, identified and controlled

Concentrated fines ignited by a carried spark, static from filter media or an ember; propagation back through ducting into the plant if not isolated.
Sparks or hot spots in the finest dust on site; explosion in the mill and its aspiration system.
Self-heating meal or hot pellets smoulder in storage; opening, disturbing or watering the vessel turns a nest into a cloud fire or explosion; entrapment during entry.
Welding, cutting or grinding without shutdown, cleaning, isolation and fire watch — direct ignition of a dust cloud or layer.

Where holding only a SWMS goes wrong
A regional mill receives wheat, barley and canola meal by truck, grinds through a single hammer mill into a batch mixer with liquid fat addition, and pellets through one press with a counterflow cooler into four finished-feed bins and a bagging line. On the first walk with Document 01 the team scores the elevator legs, the hammer mill and the collector at Extreme inherent risk, which is where the pack expects them to be. Document 02 classifies the inside of every leg, the mill housing, the collector and the bins during filling as Zone 20, the tip station and bag-change area as Zone 21, and the process floor as Zone 22 on the condition that housekeeping is held to the Document 04 standard. Document 03 then surfaces the gap: the elevator bearing sensors are fitted but wired to an alarm only, not a shutdown, and there is no hot-work permit — the last contractor welded a bracket on the mill leg with the plant running. Both go on the Document 01 action plan with owners and dates. The first Document 04 round finds dust footprints on the elevator tower floor — a Stop finding traced to a split boot seal, which is repaired the same week. Document 05 is inserted into the site emergency plan; the fire brigade is briefed on the bin layout and told that a smouldering bin is never to be opened or jetted. The residual profile after closure is High on the confined dusty plant and Medium elsewhere — which is what the pack says a well-run mill should expect.

Where the work is construction work — installation, repair, maintenance or alteration of the mill's plant and structure — the high risk construction work categories above require a Safe Work Method Statement under s 291 and s 299 of the WHS Regulation 2025 (NSW) before the work starts. The routing column of the risk assessment identifies which rows trigger that duty. The fire and explosion duties in ss 51–53 and s 355 apply to the operating mill whether or not construction work is taking place.
